Fingernail & Toenail fungus, medically known as onychomycosis, is a skin condition caused by a fungus, also commonly referred to as ringworm of the nails. These organisms grow on dead or dying tissue, often times penetrating hard to reach areas such as beneath the fingernail or toenail plate. Most of the patients we see at Hogue Cosmetic Surgery for toenail fungus concerns are found to also have Athlete’s foot (tinea pedis). The chance for a long term cure of toenail fungus in the setting of underlying Athlete’s foot is dismal. This is why at Hogue Cosmetic Surgery we have taken a comprehensive, aggressive approach to eliminating fungus and fungal spore from our patients’ feet and footwear.
Treatment options for fingernail/toenail fungus include home remedies, over-the-counter topical treatments, oral antifungal medications, and surgical removal of the infected nail. Home remedies and topical treatments, along with oral antifungal agents have low effectiveness and low cure rates. In the case of oral antifungal medications, potentially worrisome side effects discourage their use by safety-minded individuals. Recently, laser technology which penetrates and destroys fingernail & toenail fungus within and beneath the nail plate and nail bed has emerged.
At your initial visit to Hogue Cosmetic Surgery for evaluation and treatment of fingernail & toenail fungus, an examination of your fingernails and toenails is done by our physician. Photos are taken. If clinically indicated, a powerful laser is used to heat your fingernails and/or toenails up to 118-122 degrees Fahrenheit (48-50 Celsius). Three consecutive passes are done using the FDA cleared Sciton Joule ClearSense™ Laser which delivers 1064-nm Nd:YAG laser energy for targeted heating of the fingernails & toenails. While patients feel the heat on their nails during the laser treatment, surface temperatures are monitored using an infrared non-contact temperature device to ensure the temperatures are safely reached at levels sufficient to kill the fungus.
After receiving an initial series of three fingernail/toenail laser treatments at 4-week intervals to eliminate your nail fungus condition, your nails will not instantly become clear, but instead, will gradually improve over months. It may take up to 9 to 18 months to completely grow out a new set of nails to slowly replace your fungal-infected ones. If you are also diagnosed with athlete’s foot in addition to having fingernail/toenail fungus, you will be recommended to use topical over-the-counter antifungal medications twice daily on your hands and/or feet to treat your athlete’s foot condition. We recommend that you purchase over-the-counter topical antifungal agents at your local drugstore such as: (1) Lamisil (Terbinafine) 1% cream; (2) Lotrimin (Butenafine) 1% cream; (3) Tinactin (Tolnaftate) 1% cream; or other suitable topical antifungal agents not mentioned above, and follow the manufacturer’s directions-for-use, which is usually twice daily. It is important to know that an associated athlete’s foot condition must also be treated and eliminated before realistically expecting toenail fungus to be eliminated, especially since both conditions are usually caused by the same fungal organism. Many individuals with nail fungus likely acquired their condition as a direct spread of their athlete’s foot from their feet and toes to their fingernails and/or toenails.
